The Care Portal is a mobile-friendly website that allows caregivers and family members to interact with the system.

Logging In

Your Service Provider will give you a web address (URL) for logging in.

  1. Go to the URL provided
  2. Enter your email address and password
  3. Click “Log In”

If you are logging into the Care Portal for the first time you will have to activate your user account to get started.

Forgot your password?

If at any time you forget your password, click the “Forgot My Password” link on the log in screen.Follow the instructions on-screen to reset your password.

Care Portal Navigation

Select a menu item to change the content displayed. Tabs at the top of the screen provide access to additional content. The menu items available will vary depending on your User Role.
